Waist-to-Height Ratio for Women
Interpreting WHtR risk zones for women, including menopause and body-shape considerations.
The WHtR thresholds โ healthy below 0.5, increased 0.5-0.55, high 0.55-0.6, very high above 0.6 โ apply equally to women and men. This is a deliberate design choice: the biological signal (central adiposity โ cardiometabolic risk) is sex-independent, unlike BMI cut-offs that historically differed.
Body shape matters more than average
Women with a naturally pear-shape body composition (fat distributed to hips and thighs) tend to have lower WHtR at any given weight and lower cardiometabolic risk. Women with apple-shape distribution (central adiposity) show higher WHtR and higher risk โ even at 'normal' BMI. This is why WHtR often catches risk that BMI misses in women.
Menopause and body composition
The menopausal transition is associated with a redistribution of body fat from the hips toward the abdomen, largely driven by declining oestrogen. Many women see their WHtR drift upward during perimenopause and the first years post-menopause without a corresponding change in scale weight. Tracking WHtR quarterly through this transition is more informative than tracking weight alone.
Pregnancy exception
WHtR is not intended for use during pregnancy โ waist circumference during pregnancy reflects the pregnancy, not cardiometabolic risk. Resume measurement after post-partum recovery.
