Waist-to-Height Ratio vs BMI — Which Predicts Risk Better?

How WHtR compares to BMI in predicting cardiometabolic risk, and when each metric is more appropriate.

BMI has been the default anthropometric marker for four decades, but its ceiling is well-documented: it treats a lean athlete and a person with high visceral fat as identical if their weight and height match. Waist-to-height ratio (WHtR) closes that gap by measuring where fat is stored, not just how much of it there is.

Where BMI still wins

BMI remains useful at population scale for tracking obesity prevalence, allocating public health resources, and setting eligibility for weight-management interventions. It is also the only metric with 40+ years of longitudinal outcome data, which matters for insurance underwriting and legacy clinical guidelines.

Where WHtR wins

For individual cardiometabolic risk assessment, WHtR is stronger. It correctly flags central adiposity in normal-weight individuals (the so-called 'skinny fat' phenotype), works consistently across ethnicities without population-specific cut-offs, and is easier to measure at home than DEXA or bio-impedance.

The pragmatic answer

Use both. If they agree, act on the agreed picture. If they diverge — normal BMI, elevated WHtR — trust the WHtR reading for cardiometabolic decisions and consult a clinician for context.
